Summer Perk Card: Earn one stamp per qualifying transaction. One transaction per medical customer per day. A qualifying visit requires a minimum $20 spend after discounts applied. After five qualifying visits, customers will unlock a reward for $75 off a $150 purchase. The $150 minimum purchase is calculated after discounts are applied.
