There is also a $50 state registration fee payable online with checking account or by mailing a check or money order unless applying for the fee waiver.

4. Debit or credit card for physician consultation fee paid directly to Elevate Holistics.
5. Checking account and routing number to pay $50 state registration fee online via ACH payment or you can mail a check or money order to the state.
